A form signed by a doctor stating exactly what medicine or treatment a school or other provider may give, and when.

ကျောင်း သို့မဟုတ် အခြားဝန်ဆောင်မှုပေးသူက မည်သည့်ဆေး သို့မဟုတ် ကုသမှုကို မည်သည့်အချိန်တွင် ပေးရမည်ကို အတိအကျ ဖော်ပြထားသော ဆရာဝန်လက်မှတ်ပါ ပုံစံ ဖြစ်ပါသည်။
